Understanding Cervicogenic Headache post-MVA
Also known as: Neck-Related Headache, CGH, Whiplash Headache, Post-Accident Headache Cervicogenic headache is head pain originating in the upper cervical spine (C0-C3) and referred into the head via the trigeminocervical nucleus — the brainstem relay where upper neck nerves and the trigeminal nerve converge. After an MVA, the facets, muscles, and ligaments of the upper neck are routinely injured, and the resulting dysfunction produces headaches that can easily be mistaken for migraine or tension-type headache. The distinguishing clinical clue: the cervical flexion-rotation test — reduced rotation below roughly 32 degrees on the painful side — is highly suggestive of upper cervical involvement. A one-sided headache starting at the base of the skull, spreading forward to the temple or behind the eye on the same side, provoked by neck movement or pressure on the upper neck, is the characteristic presentation.
The headache "comes out of my neck" is how patients often describe it, once they learn to pay attention to the pattern. Strong evidence supports upper cervical manipulation and mobilization for cervicogenic headache. Our approach includes specific C0-C3 manipulation or mobilization (including Mulligan SNAGs), craniocervical flexion training, suboccipital release, postural correction, and ergonomic coaching. We document headache frequency, intensity, and cervical range of motion at each visit using validated tools.
Most cervicogenic headaches improve substantially over four to twelve weeks. We may recommend: diversified adjustments, Activator, cervical traction, myofascial release, trigger point therapy, Class IV laser, corrective exercise Seek immediate care if: You have a sudden "thunderclap" headache, headache with fever and stiff neck, new neurologic symptoms, or a headache that is truly the worst of your life — these require emergency evaluation.
